Abstract Rare or orphan diseases have played an important role in the pharmaceutical industry. On other hand, impact of new technologies derived from genomic research has been growing this industry, with drugs being launched on market at unsustainable prices for health systems and patients. This double tendency poses challenges to public policies Health Technology Assessment, whose hegemonic rationale is based cost-benefit analysis between therapies. The very high these require revisiting recent negotiations Brazilian Ministry Novartis regarding a possible risk-sharing agreement incorporation drug Zolgensma opportunity revisitation.
